If you pay by credit card, LPF asks that you cover the 3% merchant fee. LPF is a nonprofit organization, and your sponsorship supports our
mission. We appreciate your help with this fee so that we can maximize your support

Sponsor/Exhibitor Responsibilities.

To the fullest extent permitted by law, the organization/person described as the “sponsor” in this
document hereby assumes full responsibility and agrees to indemnify, defend, and hold harmless the Limb Preservation Foundation “LPF”
and the Marriott Denver West “hotel” from and against any and all claims or expenses arising out of Sponsor’s use of the hotel premises.
Sponsor shall obtain and maintain Comprehensive General Liability Insurance, including contractual liability covering the Sponsor’s indemnity
obligations in this clause. Such insurance shall be in the amount of not less than $1 million combined single limit for personal injury and
property damage. The hotel and LPF shall be named as additional insured on such policy, and the Sponsor shall supply the hotel with a
certificate of insurance at least 30 days prior to the event. The Sponsor understands that neither LPF nor hotel maintain insurance covering
sponsors property and it is the sole responsibility of the sponsor to obtain such insurance.

Exhibit Space.

Space assigned to the sponsor is for their exclusive use. Sponsor shall not assign or sublet any space allocated to them without
the consent of LPF and any attempted assignment shall be null and void. LPF has the right to reassign exhibit location if deemed necessary.
LPF sponsors may purchase additional booths commensurate with the value of symposium exhibit spaces.

Booth Restrictions.

Any portion of the exhibit that in the sole judgement of LPF obstructs the view, interferes with the privilege of other
sponsors/exhibitors, extends beyond the designated exhibit space, or for any reason becomes objectionable, must immediately be modified
or removed by the sponsor/exhibitor at the event.

Set-up/Dismantle.

Each sponsor will have their exhibit in place before the opening of the event and will have their exhibit dismantled and
removed after the closing hours of the event. The sponsor is responsible for the delivery of their equipment and display material.
For good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the undersigned on the contact
information section of the commitment form hereby agrees to and accepts the foregoing terms and conditions.
