    Patient Assistance

The Patient Assistance Program provides assistance to qualifying uninsured or underinsured patients who are in active treatment of their extremities for tumor, trauma or infection through six different funds. While we would like to assist patients across the United States, we are currently only providing assistance to patients residing in the nine-state Rocky Mountain Region.
(Rocky Mountain Region - Arizona, Colorado, Idaho, Kansas, Montana, Nebraska, New Mexico, Utah and Wyoming)

Patients must work with a social worker or other health care professional to complete the Patient Assistance Application.

The Bloom Again Fund

The Bloom Again Fund is a new program of The Foundation.  The Fund has been created in honor of Molly Bloom and her wish to positively benefit others who fund themselves in a situation similar to Molly's.

The Fund awards grants from equipment or opportunities for individuals who have encountered serious illness or injury and want to enhance their recovery through physical activities and sport.

Extremity Scholarship Program

Are you a current high school senior or in college and have been an extremity patient? If yes, we encourage you to apply for our Extremity Scholarship. The next application process will be in April 2017.
